Appointments & Cancellations

What is your cancellation policy?

If you need to cancel or reschedule your appointment, we ask that you notify our office at least 24 hours in advance. Appointments cancelled with less than 24 hours' notice or missed without notice may be subject to a $30 No-Show/Cancellation Fee, if determined medically appropriate by the physician

Medications & Refills

When should I request a refill?

Please request prescription refills at least 72 hours before your medication is expected to run out, especially before weekends and holidays.

What if I submit my refill request before noon?

Refill requests received before 12:00 PM (noon) on a business day are generally processed on the same day. Requests received after 12:00 PM may be processed the following business day

Urine Drug Screening & Lab Testing

Why do I need a urine drug screen?

Urine drug screening helps ensure medications are being used safely and appropriately and is part of responsible pain management.

Will my urine sample be tested on site?

Not always. At the physician's discretion, urine drug screen specimens may be sent to an independent third-party laboratory for testing.

Will I receive a bill for laboratory testing?

Possibly. Laboratory testing is performed by an outside laboratory, which may bill your insurance and/or you directly according to your insurance benefits.

Billing, Insurance & Payments

When are copays due?

Copays, deductibles, coinsurance, and any patient responsibility are due at the time of service.

Why is my copay different than expected?

Your copay is determined by the eligibility information received from your insurance company on the date of service. If you believe the amount is incorrect, our staff will gladly review your benefits with you, or you may contact your insurance carrier for confirmation.

What if I have not met my deductible?

If your deductible has not been met, you may be responsible for additional charges based on your insurance benefits.

Am I responsible if my insurance doesn't pay?

A. Yes. Insurance is a contract between you and your insurance company. Patients are ultimately responsible for services that are:

  • Not covered by insurance

  • Applied toward deductibles or coinsurance

  • Denied by the insurance carrier

  • Determined to be not medically necessary by the insurance company

What if my insurance information changes?

A. Please notify our office immediately if your insurance coverage changes. Failure to provide updated insurance information may result in delays in billing and additional financial responsibility.

How long does insurance take to process claims?

Most insurance carriers process claims within 30 to 45 days, although processing times may vary.

Do you offer payment plans?

You can set up your own payment plan via Cherry Pay. No in-house finance.

Can I use a credit card?

Yes. We accept most major credit cards.

What happens if my account becomes delinquent?

Accounts with unpaid balances beyond 30 days will be referred to collections.

Medical Records & Imaging

How can I access my medical records?

Patients have secure access to their medical records through our Patient Portal, which is our primary and preferred method for releasing records. The portal provides convenient, secure, and 24/7 access to your health information, including visit summaries, test results, and other available records.

How do I request medical records?

Patients may submit a signed Authorization for Release of Medical Records form. You may limit the information you authorize us to release by indicating specific restrictions on the authorization form.

Does your office keep imaging CDs?

No. We do not store imaging CDs. Patients are responsible for retrieving their imaging CDs promptly after they are made available.

Communication & Privacy

Can I authorize someone else to discuss my medical information?

Yes. During registration, you may authorize your spouse, family member, caregiver, or another individual to receive information regarding your care.
